Calvary Assisting Resource Emergency (C.A.R.E.) 

The undersigned understands that any assistance from Calvary Christian based  Center will be given based upon the verification of information given and the truthfulness thereof as well as verification that an emergency and/or need exist. Money given will come from tithes and offerings of Calvary Christian Center members.
